Home Medicines Review (HMR)

Patients

A Home Medicines Review (HMR) is a comprehensive review of your medicines where a pharmacist will come to your house and review your medicines. A detailed report will be sent to your doctor after the review and you will then meet with the doctor to discuss the recommendations in the report.

General Steps of a HMR for Patients

  1. The prescriber (usually your GP) will send a referral to a consultant pharmacist.

  2. The pharmacist will contact you to arrange an appointment.

  3. The service is conducted in your home and there is no fee for this service (provided you are eligible).

  4. The review will take around 1 hour, where the pharmacist can look for any possible problems with your medicines, such as side effects or drug interactions, ensure you are taking them optimally and review your goals.

  5. Afterwards the pharmacist will send a report to the doctor and you will have a follow up appointment with your doctor to discuss the report and make a medication management plan.

A Home Medicines Review (HMR) is a collaborative, comprehensive review of your patients medicines designed for patients not meeting therapeutic goals.

Many patients may benefit from a Home Medicines Review including those taking multiple medications, with multiple prescribers, recently discharged from hospital, on high risk medicines, difficulty managing medicines, suspected side effects from medicines + many other patients.

General Steps of a HMR for Prescribers

STEP 1: Determine eligibility, obtain patient consent
STEP 2: Send Referral for a HMR [Medical Objects Provider ID: SC4803000G6 or sofia.pharmacist@outlook.com]
STEP 3: Consultant pharmacist conducts review at patients home.
STEP 4: Pharmacist sends report to prescriber. Prescriber collaborates with pharmacist and discusses recommendations.
STEP 5: Medication Management Plan - Patient has HMR appointment with GP and together develops a Medication Management Plan. Plan is sent to pharmacist.
